A carried physician certification, not a state-issued permit — but unlike West Virginia's affidavit it must follow "the manner and format required by the Automotive Safety Enforcement Division of the Department of State Police", and only a physician licensed IN MARYLAND may sign. Duration is set by the physician's judgment: valid for the period the physician determines the owner needs the tint, "not to exceed 2 years", or INDEFINITELY where the physician determines the condition is permanent ((i)(4)(ii)).
